Patient:

4 yo, female, spayed, DSH cat.

History:

Presented with a history of vomiting and inappetence for 5 days. The owner saw her playing with her flip flops 5 days ago.

Findings:

On the ultrasound, there were small intestinal loops dilated with fluid and a thickened portion of the SI was noted. In the lumen, multiple small foreign bodies were detected. There was small amount of ascites.

Diagnosis:

The foreign bodies were small plastic pieces of flip flop.

Discussion:

There are studies that have shown that ultrasonography has a higher specificity and specificity than Xrays on determining an intestinal obstruction.(1,2) In this case the lumen of the small intestine was less than 1.5cm but more than 1cm. (2) This could be because the obstruction of the alimentary tract was not complete.

Ασθενής:

Θηλυκή, στειρωμένη, γάτα 4 ετών

Ιστορικό:

Προσκομίσθηκε λόγω συχνών εμέτων και ανορεξίας, συμπτώματα τα οποία επέμεναν  επί πέντε ημέρες παρά την συμπτωματική θεραπεία.

Ευρήματα:

Στον υπέρηχο βρέθηκε τμήμα του λεπτού εντέρου παχυμένο, στον αυλό του οποίου υπήρχαν μικρά ξένα σώματα τα οποία δημιουργούσαν ακουστική σκιά. Το αρχικό τμήμα του λεπτού εντέρου ήταν διατεταμένο με υγρό περιεχόμενο.

Διάγνωση:

Στο χειρουργείο αφαιρέθηκαν μικρά τμήματα πλαστικού τα οποία προέρχονταν από σαγιονάρες.

Συζήτηση:

Υπάρχουν μελέτες που αποδεικνύουν πως ο υπέρηχος έχει μεγαλύτερη ευαισθησία και ειδικότητα στο να εντοπίζει την έμφραξη του λεπτού εντέρου σε σχέση με την απλή ακτινογραφία.(1,2) Σε αυτό το περιστατικό η διάμετρος του αυλού του λεπτού εντέρου ήταν μικρότερη από 1.5 εκ. αλλά μεγαλύτερο του 1 εκ.(2) Αυτό μπορεί να συνέβαινε λόγω της μερικής και όχι της πλήρους έμφραξης του πεπτικού σωλήνα.
